Combined Decongestive Therapy / Manual Lymphatic Drainage (CDT/MLD)
Combined Decongestive Therapy (CDT) is the gold standard treatment for lymphedema. It combines several techniques—Manual Lymphatic Drainage (MLD), compression therapy, therapeutic skin care, and remedial exercises—delivered in two phases:
- Intensive Phase: Focused on reducing swelling and lymphatic volume.
- Maintenance Phase: Supports long-term management and prevention of complications.
Developed by Dr. Emil Vodder, this comprehensive approach:
- Stimulates lymph flow
- Reduces swelling and discomfort
- Improves mobility
- Prevents fluid buildup and infections (e.g., cellulitis)
- Enhances overall quality of life

Key Components of CDT
Manual Lymphatic Drainage (MLD)
A gentle, skin-stretching technique using rhythmic, circular, and spiral movements to stimulate lymph flow.
The Dr. Vodder Method is a specialized and widely recognized form of MLD.
Compression Therapy
- Phase 1: Bandaging to prevent fluid from reaccumulating.
- Phase 2: Custom-fitted compression garments to maintain progress.
Remedial Exercises
Gentle range-of-motion movements performed with compression to:
- Activate muscle and lymphatic pumps
- Promote lymphatic circulation
- Strengthen the lymphatic system
Therapeutic Skin Care
Essential for infection prevention. Emphasis is placed on:
- Proper hygiene
- Moisturization
- Monitoring for early signs of skin complications (e.g., cellulitis)
